Review the descriptions below and decide which option you desire. Then complete the application on the next page and submit it online (credit card payments only). That's all there is to it!

Your coverage is processed and put into effect the day we receive your payment and within 4 to 6 weeks, you will receive your evidence of coverage certificate for a 12 month term showing the coverage you chose.

The coverage extends throughout the United States and its possessions (although not to cruise ships). Unfortunately coverage cannot be changed in mid term so choose your option well. There are no flat cancellations.

Coverage is available only to FSMTA members who are licensed, registered, or certified massage therapists. If you are not a member of the FSMTA, check out the membership application on the web site and submit online (with credit card payments only) or print out the completed form and mail in with your payment. fill out member application

Professional Liability Including Comprehensive General Liability - ($99/$115)

General liability coverage protects you from liability arising out of bodily injury or property damage from your premises or operations (slip and fall coverage) or from your products (manufactured products you resell as well as oil/lotions you mix for use with your clients) while practicing as a licensed massage therapist as defined by Chapter 480. Included in this coverage is professional liability which covers liability arising out of any act, error or omission in rendering or failure to render professional services as a licensed massage therapist as defined by Chapter 480; however, it does not include physical/sexual misconduct. This policy will defend you for alleged wrongdoing in any of these areas. Your coverage is subject to the terms, conditions and exclusions in the Master Policy, which can be purchased for a handling fee of $10.
This coverage is what is necessary for an establishment license.

Limits of Coverage: $1,000,000 each occurrence

FAQS

Here are some of the most frequently asked questions in regards to insurance:

How long will it take for me to get my proof of insurance?

There is a delay between when insurance goes into effect and when you receive your evidence of coverage. This delay is anywhere between 2 to 6 weeks. It takes the insurance company time to process the numerous applications we send them everyday, so if you don’t receive your evidence of coverage right away, please do not be concerned. If you are going to open an establishment, please plan for this delay. You are covered from the day we process your payment.

I am applying for an establishment license. Can I get the required insurance through the FSMTA? What coverage option do I need to take? Is there anything else I need to know so that there won’t be any problems with getting my establishment license?

Our insurance program meets the requirements for the Establishment License. All you are required to have is the $1,000,000/$1,000,000 Professional Liability Insurance. However, you do need to make sure that your evidence of coverage matches your establishment license application from the name of the business to the address. These items must be the same or it is likely that your application for a license will be sent back to you. Additionally, there is a form in the application for an establishment license that says the insurance agent needs to sign it. If you attach a copy of your evidence of coverage to the form, you will not need to have the form completed.

My insurance expires on 4/1/02. Will I get something in the mail so that I can renew it?

Yes. We send out a renewal notice about 45 days before your expiration date for you to renew. A final notice will be sent out around 15 days before your expiration as well. If you don’t receive one, you may want to call the Central Office and make sure that your correct address is on file.

My insurance expires on 4/1/02. When is it actually due? Is there a grace period?

There is an expiration date on your policy and this date is firm. In order for you not to have a lapse in coverage, you need to make sure that we have your renewal insurance application in our office BEFORE the expiration date. The Insurance Company offers no grace period for insurance. If we receive it after the date, your new policy will be issued with a new effective date (the date we processed your payment) and you may have several days where you were not covered. This is a requirement from the insurance company.

Is there anything that might hold up processing my insurance application?

Actually, there is. You need to sign and initial your insurance application or renewal notice (does not apply to online applications).

What do I do if I need to add someone onto my policy or correct my address? Is there a charge?

You can add additional insureds or change your address at any time. Just call the Central Office and give us the information, and we will contact the insurance company for you. The first two additional insureds are $25.00 each, all others are free. There is no charge for address/information changes.

My insurance expires on 4/1/02 and my membership expires on 3/31/02. Can I send in my insurance payment before my membership or should I send them in together?

In order to apply for or renew your insurance, you must be a current member of the FSMTA. If you are renewing your insurance, and your membership and insurance expire within a day of each other (i.e. membership expires 3/31/02 and your insurance expires 4/1/02), you must renew your membership in order to renew your insurance. On the date your insurance coverage year starts, you must be a current member. This means that if you send in your 4/1/02 insurance renewal in March but you haven’t sent in your 3/31/02 membership renewal, we cannot process your insurance renewal form until we receive your dues. If your membership and insurance expire around the same date, you need to renew your membership in order to renew your insurance. Insurance is a benefit of being a current member.

I'm not comfortable submitting my credit card over the internet. Is there any other way to get the insurance?

Absolutely! You can call the Central Office at 877-376-8248 to have an application mailed to you. To have a copy faxed to you, please call 888-553-7796.

If I am sending in my payment for my membership and insurance, do I need to send two checks?

Yes. Membership is payable to Florida State Massage Therapy Association, Inc. and the insurance is payable to FSMTA Association Services, Inc.
