Now you can renew your membership online, participate in the Therapist Locator, add modalities, change addresses and phone numbers, and apply for new liability insurance. Here are instructions for accessing your FSMTA member account:
1. Log onto www.fsmta.org
2. On the left side of the home page, click Members Log-In. This takes you to the MyFSMTA Login page.
3. If you are already an FSMTA member, you will have a username and password. DO NOT create a new MyFSMTA Account! This will create a duplicate record for you. Your username is by default your email address. The computer automatically assigns you a password, so the first time you log on, you will need to click the Don't Know Your Password? Just Click Here link, enter your email address and your password will automatically be emailed to you. If for any reason your email address is NOT in our database, then you will be prompted to call us and we will give you that information over the phone. Once you log in the first time, you will be able to change your username and password so that it's easy for you to remember.
4. Once you log in, you will see a message like this:
Welcome to the MyFSMTA Secure Portal, Lynn Your membership is current through: 00/00/00. From this screen you can renew your membership, apply for new liability insurance AND view your profile. If you click on View Profile at the top of the page, you will see your information as it appears in our database. There are links to change your username and/or password, and also a link to update your profile. When you click on Update Profile you will be able to change/update your contact information, opt in or out of the Therapist Locator function, and manage your modalities

FSMTA FYI - Does the BOMT have your correct address Renewal Information for the Board of Massage Therapy?
A notice for renewal is mailed to each licensee at the last address provided to the Board. Failure to receive this notification does not relieve the licensee of the responsibility of meeting the renewal or continuing education requirements.
Q: Can I fax my change of address to you or may I leave my change of address on your voice mail or can you take a change of address over the telephone?
A: Because of recent changes in Florida law, you now have the option of submitting your change of address using our _electronic change of address form_ (http://www.doh.state.fl.us/mqa/chng_address.htm) or you may write a letter requesting the change that includes the type of address to be changed and faxing it to (850) 487-9626. You will need your current license number and the control number from the license for security reasons. Remember a change of location for a massage establishment requires the completion of an application and submission of the appropriate fees.
